This private clinic was losing 20–30 minutes per patient visit to paper-based intake forms. The app replaced 5 distinct paper documents with a single, guided digital flow that patients complete on a provided tablet before their appointment. Form data is written directly to Firestore and accessible to the clinic's admin panel in real time, eliminating manual data entry. Conditional logic in the form builder shows or hides questions based on previous answers, keeping the experience concise for each patient.
